Responsibilities

  • Provide direct patient care and support under the supervision of registered nurses.

  • Engage in continuous learning and professional development opportunities.

  • Administer medications and monitor patient responses to treatments.

  • Adhere to safety and infection control protocols at all times.

  • Utilize performance metrics to track personal and team success.

  • Assist in developing and implementing patient care plans.

  • Maintain accurate patient records and documentation.

  • Monitor vital signs and report any changes to the nursing staff.

  • Communicate effectively with patients and their families regarding care.

  • Participate in team meetings to discuss patient progress and care strategies.

  • Assist with daily living activities to enhance patient comfort.


Experience/Qualifications

  • Current LPN license in the state of practice.

  • Strong commitment to continuous learning and professional development.

  • Experience in direct patient care, preferably in a clinical setting.

  • Excellent communication skills with patients and families.

  • Willingness to engage in performance metrics and accountability.

  • Ability to administer medications and monitor patient responses accurately.

  • Familiarity with safety and infection control protocols.

  • Ability to work collaboratively in a team environment.

  • Proficient in maintaining accurate patient documentation.

  • Experience in developing patient care plans is a plus.
