Job Summary: As a Licensed Practical Nurse (LPN) in our Long Term Acute Care facility, you will play a vital role in providing comprehensive nursing care to patients with complex medical conditions. You will work closely with a team of healthcare professionals to ensure the physical and emotional well-being of our patients. Your dedication to patient-centered care, attention to detail, and clinical expertise will be essential in promoting positive patient outcomes. Key Responsibilities: Patient Assessment and Monitoring: Conduct thorough assessments of patients' health conditions, including vital signs, symptoms, and medical histories. Continuously monitor patients' progress and respond promptly to changes in their condition. Medication Administration: Administer medications and treatments as prescribed by physicians, ensuring accurate dosages, routes, and timing. Maintain accurate medication records and assist in medication reconciliation. Wound Care: Provide wound care and dressing changes for patients with complex wounds. Monitor and report any signs of infection or deterioration in wound status. Collaborative Care: Work closely with the interdisciplinary healthcare team, including physicians, registered nurses, therapists, and other healthcare professionals, to develop and implement comprehensive care plans. Patient Education: Educate patients and their families on medical conditions, treatment plans, and self-care techniques. Provide emotional support and address any questions or concerns. Documentation: Maintain accurate and thorough electronic health records (EHR) of patient assessments, interventions, and outcomes. Ensure compliance with all legal and regulatory requirements. Infection Control: Follow strict infection control protocols to prevent the spread of infections within the facility. Adhere to established safety procedures and protocols. Emergency Response: Respond effectively to medical emergencies within the facility, including initiating CPR and other life-saving measures as necessary. Quality Assurance: Participate in quality improvement initiatives and assist in data collection and analysis to enhance patient care and outcomes. Qualifications: Valid and current Licensed Practical Nurse (LPN) license and able to work in the state of Indiana. American Heart Association BLS (Basic Life Support) in-person certification required. Strong clinical assessment skills Excellent communication and interpersonal abilities Attention to detail and strong organizational skills Ability to work collaboratively within a healthcare team Min 1yr recent experience in long-term acute care or critical care setting Benefits: Competitive salary and benefits package.
