LPN - LTC

Job Summary

The LPN LTC provides nursing care to residents in a skilled nursing facility (SNF) or long-term care setting. The nurse monitors residents' health, administers medications, assists with daily care, documents patient information, and coordinates with registered nurses and other healthcare professionals.

Key Responsibilities

• Provide nursing care to residents in a long-term care facility.

• Administer medications and treatments as prescribed.

• Monitor vital signs and changes in residents' health.

• Document nursing care using the MatrixCare charting system.

• Assist residents with daily care needs and promote safety.

• Communicate with physicians, RNs, and other healthcare staff.

• Follow infection-control procedures and facility policies.

Requirements

• Active New York State LPN license mandatory.

• BLS certification mandatory.

• Minimum 2 years of LPN experience in a skilled nursing facility or long-term care setting.

• Previous travel nursing experience required, preferably with contract renewals.

• Experience with MatrixCare charting preferred.
