About the Role:Candis Inc in Roanoke, VA is looking for a dedicated and compassionate Licensed Practical Nurse (LPN) to join their growing healthcare team. This is an exciting opportunity to make a real difference in patients' lives while advancing your nursing career in a supportive environment.
Responsibilities:- Provide direct patient care including monitoring vital signs, administering medications, and documenting health status
- Assist in developing and implementing individualized patient care plans
- Collaborate with RNs, physicians, and interdisciplinary team members to ensure quality care
- Perform wound care, catheter care, and other clinical nursing procedures
- Educate patients and families on health conditions, treatments, and discharge instructions
- Accurately maintain and update electronic health records (EHR) and patient documentation
- Respond promptly to patient needs and changes in condition
Requirements:- Active LPN license in the state of Virginia (or eligibility to obtain)
- Graduate of an accredited Licensed Practical Nursing program
- Current CPR/BLS certification
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills with patients and care teams
- Ability to work independently and in a team-oriented environment
- Prior LPN experience preferred but new graduates are encouraged to apply
- Experienced in Mental Health/Behaviors
