Responsibilities:
Provide direct patient care including monitoring vital signs, administering medications, and wound care
Document patient information, observations, and care activities accurately in medical records
Communicate effectively with patients, families, and interdisciplinary care teams
Perform routine clinical procedures and respond promptly to patient needs
Educate patients and caregivers on health management, medications, and discharge instructions
Maintain a safe, clean, and compliant care environment in accordance with regulatory standards
Requirements:
Active LPN license in the state of New Jersey (required)
Graduate of an accredited Licensed Practical Nursing program
Current BLS/CPR certification
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with patients and staff
Ability to work independently and collaboratively in a fast-paced environment
Prior clinical experience in a similar healthcare setting preferred
