    Job Description

LVN Care Coordinator – Home Health | Oxnard, CA

Pay: $35–$45 per hour, depending on experience
    Schedule: Monday–Friday, 9:00 AM–5:30 PM
    Work Arrangement: Primarily office-based, in-person
On-Call: One seven-consecutive-day rotation per month

    About the Opportunity

An established home health provider is seeking a Licensed Vocational Nurse (LVN) Care Coordinator to support patient care coordination, clinical oversight, and quality assurance. This role works closely with clinical staff and leadership to promote continuity of care, maintain regulatory compliance, and ensure patients receive appropriate, high-quality services.

Although primarily office-based, the position includes occasional patient visits to support urgent clinical coverage and staffing needs.

    Key Responsibilities

    • Review patient referrals, clinical information, diagnoses, medications, and treatment plans to coordinate appropriate care.
    • Assign clinicians based on patient needs and support individualized plans of care.
    • Collaborate with clinical staff and leadership to address patient care concerns and promote continuity of services.
    • Provide clinical guidance and support to personnel during regular operating hours.
    • Conduct quarterly clinical record reviews to evaluate documentation, quality of care, and regulatory compliance.
    • Identify improvement opportunities and communicate findings to the Clinical Director.
    • Assist with staff recruitment, onboarding, orientation, and continuing education.
    • Participate in case conferences, quality initiatives, and organizational activities.
    • Conduct PRN patient visits when needed due to urgent coverage needs, staffing limitations, or scheduling gaps.
    • Complete required assessments, documentation, and follow-up for assigned patient visits.

    Patient Visit & Travel Expectations

This is primarily an office-based role, and routine field visits are not expected. However, LVN Care Coordinators may be assigned patients within their local county when urgent clinical coverage is needed.

When temporary coverage is required, the coordinator may continue caring for the assigned patient until discharge or until appropriate coverage becomes available. Unlike regular field nurses, this position does not typically require routine travel within a designated mileage radius.

    Additional requirements:

    • Participate in one full seven-day on-call rotation each month, with scheduling flexibility coordinated among the LVN Care Coordinators.
    • Work overtime as needed to support operational requirements.
    • Attend annual in-service training at the organization's headquarters in Montrose, California.

    Required Qualifications

    • Current, active California Licensed Vocational Nurse (LVN) license.
    • At least two years of home health case management experience within the past five years.
    • Current CPR certification, maintained throughout employment.
    • Knowledge of patient care coordination, clinical assessment, and quality management.
    • Strong organizational, time-management, communication, and problem-solving skills.
    • Ability to provide clinical guidance, collaborate with healthcare professionals, and maintain compliance with clinical standards and organizational policies.
    • Willingness to participate in the monthly on-call rotation and provide patient coverage as needed.

    Compensation & Benefits

    • $35–$45 per hour, depending on experience.
    • 401(k) retirement plan
    • Health insurance
    • Dental insurance
    • Paid time off (PTO)
    • Flexible schedule
