Job Title: Licensed Vocational Nurse (LVN) – Home Health
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide skilled nursing care and related services to patients in a home health setting under the direction of a Registered Nurse (RN) or physician. The LVN will assist in implementing individualized care plans, perform clinical procedures, administer medications, educate patients and families, and promote safe, high-quality patient care.
Active Licensed Vocational Nurse (LVN) license in the state of practice
Current Basic Life Support (BLS/CPR) certification
High School Diploma/GED preferred
Bachelor's Degree or equivalent combination of education and experience preferred
Valid Driver's License and reliable transportation preferred
Arrhythmia certification preferred
Strong clinical, communication, organizational, and patient education skills
Home health or clinical nursing experience preferred
Provide skilled nursing care under the supervision of a Registered Nurse (RN) or physician.
Perform home visits and deliver patient care according to established care plans and organizational policies.
Administer medications and perform physician-ordered treatments and clinical procedures.
Assist in developing and implementing individualized patient care plans.
Monitor patient conditions and promptly report changes to the supervising RN or physician.
Educate patients and caregivers on medications, treatments, disease management, and home care.
Perform clinical procedures such as phlebotomy, injections, wound care, and specimen collection as appropriate.
Maintain accurate, timely, and compliant clinical documentation.
Maintain medical supplies and equipment while ensuring a clean and safe work environment.
Participate in on-call responsibilities as assigned.
Collaborate with interdisciplinary healthcare teams to ensure continuity of patient care.
Perform other job-related duties as assigned.
