Job Title: Licensed Vocational Nurse (LVN) – Home Health

Job Summary:

We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to provide skilled nursing care and related services to patients in a home health setting under the direction of a Registered Nurse (RN) or physician. The LVN will assist in implementing individualized care plans, perform clinical procedures, administer medications, educate patients and families, and promote safe, high-quality patient care.

Requirements:

  • Active Licensed Vocational Nurse (LVN) license in the state of practice

  • Current Basic Life Support (BLS/CPR) certification

  • High School Diploma/GED preferred

  • Bachelor's Degree or equivalent combination of education and experience preferred

  • Valid Driver's License and reliable transportation preferred

  • Arrhythmia certification preferred

  • Strong clinical, communication, organizational, and patient education skills

  • Home health or clinical nursing experience preferred

Key Responsibilities:

  • Provide skilled nursing care under the supervision of a Registered Nurse (RN) or physician.

  • Perform home visits and deliver patient care according to established care plans and organizational policies.

  • Administer medications and perform physician-ordered treatments and clinical procedures.

  • Assist in developing and implementing individualized patient care plans.

  • Monitor patient conditions and promptly report changes to the supervising RN or physician.

  • Educate patients and caregivers on medications, treatments, disease management, and home care.

  • Perform clinical procedures such as phlebotomy, injections, wound care, and specimen collection as appropriate.

  • Maintain accurate, timely, and compliant clinical documentation.

  • Maintain medical supplies and equipment while ensuring a clean and safe work environment.

  • Participate in on-call responsibilities as assigned.

  • Collaborate with interdisciplinary healthcare teams to ensure continuity of patient care.

  • Perform other job-related duties as assigned.
