About the Role
STATIM CARE HOSPICE INC is seeking a compassionate and dependable
Registered Nurse (RN) or Licensed Vocational Nurse (LVN) to provide hospice home visits for a patient located in
California City, CA.
This is an excellent opportunity for a nurse who is comfortable working independently in the field and has a heart for providing compassionate, patient-centered end-of-life care. The nurse will work closely with our hospice interdisciplinary team to ensure the patient receives appropriate skilled care, symptom management, education, and support in the home.
Responsibilities
- Provide scheduled skilled nursing visits to a hospice patient in California City
- Perform nursing assessments within the scope of the RN or LVN license and hospice plan of care
- Monitor the patient's condition and promptly report significant changes to the hospice team and physician as appropriate
- Monitor pain, respiratory status, comfort, medication effectiveness, and other symptoms
- Provide medication and treatment education to the patient, family, and caregivers
- Reinforce hospice education regarding disease progression, comfort measures, safety, and when to contact hospice
- Perform treatments and other skilled nursing interventions as ordered and within scope of practice
- Coordinate care and communicate effectively with the RN case manager, hospice physician, and interdisciplinary team
- Complete accurate, thorough, and timely clinical documentation following each visit
- Maintain compassionate and professional communication with the patient and family
Requirements
- Active and unrestricted California RN or LVN license
- Hospice, palliative care, home health, or field nursing experience preferred
- Familiarity with hospice symptom management and end-of-life care preferred
- Strong nursing assessment, communication, and documentation skills
- Ability to work independently in a patient's home
- Reliable transportation and valid California driver's license
- Dependable, compassionate, and comfortable working with patients and families facing end-of-life care
- Ability to communicate promptly with the hospice team regarding changes in patient condition
