About the Role

We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our home health and palliative care team. In this role, you will deliver high-quality direct patient care across both home health and palliative care settings, working collaboratively under the supervision of a Registered Nurse to support positive patient outcomes. For our palliative care patients, you will play a vital role in providing comfort-focused care that prioritizes quality of life, symptom management, and meaningful support for patients and their families during some of life's most challenging moments.

Key Responsibilities

  • Provide direct patient care in both home health and palliative care settings in accordance with the California Nurse Practice Act
  • Implement and follow individualized plans of care established by the supervising RN
  • Deliver compassionate, comfort-focused palliative care with an emphasis on pain and symptom management and quality of life
  • Maintain accurate, timely clinical documentation consistent with the plan of care
  • Educate and support patients and their families/caregivers on diagnoses, treatments, self-care techniques, and palliative care goals
  • Assist RNs and physicians with specialized procedures, including equipment preparation and aseptic technique
  • Provide emotional support and sensitive communication to palliative care patients and their families, working closely with the interdisciplinary team
  • Identify and communicate patient needs for additional disciplines (HHA, OT, PT, MSW, ST, Dietitian) to the RN and/or Clinical Supervisor
  • Use supplies and equipment responsibly and efficiently
  • Participate in ongoing professional development and growth
  • Perform additional duties as directed by the supervising RN

Qualifications

  • Graduate of an accredited practical/vocational nursing program
  • Minimum of 3 years of nursing experience; at least 1 year in a community/home health or medical/surgical setting required
  • Experience in palliative care, hospice, or end-of-life care is a plus
  • Current, active LVN licensure in the State of California
  • CPR certification (must maintain current)
  • Strong verbal and written communication and organizational skills
  • Demonstrated empathy, emotional resilience, and ability to support patients and families navigating serious illness
  • Adherence to accepted professional nursing standards and practices
  • Valid California driver's license, reliable personal vehicle with state-compliant insurance coverage
