Description
Job Summary:
Experience Level: Mid-Senior (5+ to 7 years total; 6 years Case Management; 2 years supervisory/management).
Education: Master's degree in business, social work, nursing, or healthcare-related field (minimum Bachelor's noted).
Key Responsibilities:
Manage daily operations and staff in Case Management Department including care coordination, discharge planning, utilization management, social work, and pre-admission services.
Act as liaison between patients, families, staff, physicians, healthcare team, community agencies, managed care, and skilled nursing facilities.
Establish, implement, and monitor best practices and clinical standards for quality patient care and organizational financial integrity.
Required Qualifications:
Master's degree from accredited school in business, social work, nursing, or healthcare-related field.
Washington Registered Nurse License upon hire.
6 years experience in Case Management.
2 years supervisory/management experience, preferably in Case Management.
Previous nursing experience in complex client care areas.
Experience in problem-solving, conflict resolution, and adult education for multi-disciplinary staff.
Preferred Qualifications:
Certified Case Manager (CCM) or Accredited Case Manager (ACM) upon hire.
Case Management and supervisory experience in acute care hospital.
