Full-Time | 40 Hours/Week | Monday-Friday, 9:00 AM-5:00 PM
Salary: $65,000-$70,000 annually (paid biweekly)

Northeast Arc is seeking a compassionate and organized MassAbility Case Coordinator to support individuals with disabilities in living independently within their communities. This role provides direct, in-home case management and coordination services, helping participants access housing, healthcare, benefits, transportation, community resources, and other supports that promote self-determination and independence.

Key Responsibilities:

• Provide individualized support and case coordination for assigned participants.
• Assist with housing, budgeting, public benefits, healthcare appointments, transportation, and community engagement.
• Help participants achieve goals outlined in their service plans.
• Connect individuals with community resources and support networks.
• Maintain accurate documentation and program records.
• Collaborate with participants, families, providers, and funding agencies.
• Participate in an on-call rotation as assigned.

Qualifications:

• Associate's or Bachelor's degree in Human Services, Social Work, Psychology, Rehabilitation, or a related field preferred.
• Experience supporting individuals with disabilities or in human services preferred.
• Strong organizational, communication, and problem-solving skills.
• Ability to work independently and manage a caseload.
• Valid driver's license, reliable transportation, and ability to travel throughout the service area.
