    Position Overview:
    Are you a detail-oriented nurse who thrives on making a real difference in resident care? We are looking for an enthusiastic MDS Coordinator in Chicago near Edgewater area to join our dynamic team in Chicago! In this role, you’ll lead the charge in accurate assessments, coding, and documentation while collaborating with a passionate clinical team to ensure top-quality care.     Key Responsibilities:
    • Complete and coordinate all Minimum Data Set (MDS) assessments accurately and on time.
    • Ensure compliance with federal and state regulations, including CMS guidelines.
    • Collaborate with nursing staff, interdisciplinary teams, and leadership to drive quality improvement initiatives.
    • Support care planning and ensure resident-centered documentation.
    • Train and guide staff on MDS processes and best practices.

    Qualifications:
    • Current RN or LPN license in Illinois.
    • Strong knowledge of MDS assessments, coding, and regulatory requirements.
    • Excellent communication, organization, and teamwork skills.
    • Passion for resident-centered care and improving clinical outcomes.
