Schedule: Monday–Friday | 9:00 AM–5:00 PM

We are seeking an experienced Registered Nurse (RN) MDS Coordinator to join a dedicated skilled nursing team in Lockport, NY!

This is an excellent opportunity for an RN with strong MDS assessment, care planning, regulatory compliance, and leadership experience who is looking for a stable, rewarding position with an excellent work-life balance.

Key Responsibilities:

• Oversee accurate and timely MDS assessments and submissions.

• Ensure compliance with federal and NYS regulations.

• Lead interdisciplinary care planning and coordination.

• Review clinical documentation and identify opportunities for improvement.

• Collaborate with nursing leadership to support quality resident care and accurate reimbursement.

• Participate in utilization review meetings and daily clinical huddles.

Qualifications:

• Active NYS RN license required.

• Previous MDS Coordinator experience in skilled nursing or long-term care.

• Strong knowledge of MDS assessments and regulatory requirements.

• Excellent organizational, communication, and leadership skills.
