We're preparing for upcoming MDS Coordinator opportunities with skilled nursing and rehabilitation facilities on Long Island.

You'll own the MDS cycle from admission through discharge, run interdisciplinary care plan meetings, and audit documentation so the record reflects the care actually delivered.

• $110,000 to $135,000 base salary, set by the hiring facility based on experience

Who will love this role

• An RN with a current New York State license
• Prior MDS experience in skilled nursing or long-term care
