Role Details 

• Coordinate and complete accurate, timely MDS assessments and the Resident Assessment Instrument process
• Partner with nursing, therapy, social services, dietary, activities, and other disciplines to develop individualized care plans
• Monitor assessment schedules, supporting documentation, and submission deadlines
• Review clinical records for accuracy, completeness, and consistency
• Support compliance with applicable federal and state requirements, Medicare, Medicaid, and PDPM guidelines
• Participate in care plan meetings, quality improvement activities, audits, and survey preparation
• Educate and support team members on MDS documentation and process updates
• Stay current on regulatory changes and share practical guidance with the care team

What You Bring

• Current, active Registered Nurse license in [State]
• Long-term care or skilled nursing experience
• Working knowledge of MDS 3.0, RAI, care planning, and clinical documentation
• Familiarity with Medicare, Medicaid, PDPM, and reimbursement processes
• Strong organization, critical thinking, communication, and follow-through
• Ability to manage deadlines while keeping resident needs at the center of the work
• [Minimum years of MDS experience] preferred
• RAC-CT or related certification preferred, but not required
