We are seeking a compassionate Registered Nurse (RN) to serve as an MDS Enrollment RN. In this role, you'll complete in-home clinical assessments for prospective participants and collaborate with an interdisciplinary team to support the enrollment process. What You'll Do
Conduct in-home assessments and complete the Minimum Data Set–Home Care (MDS-HC)
Evaluate medical, functional, psychosocial, caregiver, and home safety needs
Present assessment findings to the interdisciplinary team
Coordinate additional clinical evaluations as needed
Serve as a clinical liaison between prospective participants, caregivers, referral sources, and the care team
Maintain accurate electronic documentation and support a seamless enrollment process
Qualifications
Active Massachusetts Registered Nurse (RN) license
Associate's or Bachelor's degree in Nursing, or completion of an accredited Nursing Diploma Program
At least 1 year of nursing experience in geriatrics, home health, long-term care, case management, community health, or adult behavioral health preferred
Strong assessment, communication, and organizational skills
Experience with electronic medical records and Microsoft Office
Valid driver's license and reliable transportation for frequent local travel
