Medicaid Planner / Case Manager (Remote – Treasure Valley Area)

Location: Remote (Must reside within 1hr driving distance of the Treasure Valley, Idaho)

 

Schedule: Monday–Friday, 9:00 AM – 5:00 PM

 

 

Compensation: Competitive base + commission opportunities

 

 

Travel: Quarterly in-person team meetings required


About the Role

 

We are seeking a driven, compassionate, and highly capable Medicaid Planner / Case Manager to join our growing team. This role blends strategic Medicaid planning with hands-on case management, guiding families through one of the most critical and complex seasons of life.

 

You do not need to be certified yet—we will train and support the right person—but you must bring relevant experience, strong critical thinking skills, and a genuine desire to help people.

 

 

This is not a clock-in/clock-out role. It’s for someone who takes ownership, communicates clearly, and thrives in a team that values growth, accountability, and positive impact.

 

This role is a unique blend of strategic planning, advocacy, relationship-building, and case management. In addition to developing Medicaid eligibility strategies, this position is responsible for leading consultations with prospective clients, educating families on available services, discussing service recommendations and fees, and helping guide families toward enrollment and active case planning.

The right candidate is someone who can build trust quickly, communicate with confidence and empathy, and help families move from uncertainty to action during emotionally difficult situations. This is a consultative role that requires strong critical thinking, emotional intelligence, and the ability to confidently guide conversations and recommendations.

Key Responsibilities

  • Conduct and and convert Medicaid planning consultations (virtually) for individuals and families
  • Analyze financial and clinical situations to develop Medicaid eligibility strategies
  • Oversee and guide case managers in executing Medicaid plans
  • Provide weekly updates to clients throughout their case
  • Maintain consistent communication with clients and families, assisted living and skilled nursing facilities, community partners, and state Medicaid agencies
  • Participate in weekly team Zoom meetings and one-on-one coaching and oversight sessions with case managers
  • Ensure all planning strategies align with current Medicaid eligibility guidelines, including A&D Waiver programs

Preferred Background & Experience

  • We are looking for candidates with experience in one or more of the following:
  • Medicaid case management or eligibility work
  • Banking or financial services (asset analysis, compliance, etc.)
  • Skilled nursing facility business office management
  • Social work, healthcare navigation, or long-term care coordination
  • Experience converting consultations into active clients through strong communication, education, and relationship-building

 

Bonus if you have:

 

  • Experience with Medicaid planning or long-term care eligibility
  • Strong understanding of financial documentation and asset structuring
  • Confidence leading client conversations, discussing service recommendations and fees, and helping families move forward with planning services
