Who We Are Looking For: 

Are you the Medical Appointment Coordinator we have been looking for? JRI’s Developing Abilities program provides opportunities for people to live in homes and apartments in their communities and to grow through job training, supported employment and day habilitation services. We provide therapeutic and social supports to enrich the lives of people with disabilities.

Compensation

The pay range for this position is $22.00 to $26.00 per hour. The listed range reflects what we reasonably and in good faith expect to pay for this role at the time of posting the position. The actual compensation an employee is offered may vary based on several factors such as experience, education, licensure, certifications and qualifications.  This range may be modified in the future. 

In this role, you will:

Healthcare

  • Coordinate physical, behavioral and mental health services for individuals
  • Provide formal training for staff on healthcare issues in coordination with nursing staff.
  • Manage the program’s health care resources, equipment, and supplies.
  • Coordinate licensing activities, program evaluation efforts, and other quality improvement projects.

Case Management

  • As a member of the support team, participate in the evaluation, referral and transition of new individuals and in the discharge planning for individuals served.
  • Prepare and provide staff training in individual support plans documents, including assessments, goals and support strategies.
  • Coordinate the acquisition and use of assistive devices and materials needed to implement support strategies.
  • Create and supervise staff in the use of data collection and documentation materials.
  • Ensure that learning strategies are integrated into individuals’ daily routines by program staff.
  • Provide initial staff training and ongoing supervision to staff on these strategies.
  • Create and maintain current health care records, including the use of web-based data formats.
  • Prepare and distribute all required progress notes and reports.
  • Secure all required consent forms and Human Rights Committee approvals for health care-related protocols.

Coordination Between Outside Providers and JRI Services

  • Plan, coordinate and schedule all physical, behavioral and mental health appointments, evaluations, and lab work for individuals.
  • Facilitate communication between individuals and their health care providers, JRI nursing, JRI clinicians, guardians, family, direct support and program staff to ensure the best possible care.
  • Accompany individuals on appointments as an advocate, informant and service coordinator.
  • Responsible for all internal and external documentation including health care assessments, appointment forms, health care records, and prior approval requests.

Oversee Medication Administration System

  • Oversee the medication administration system for individual’s served, according to DDS and DPH regulations and JRI/DA policies and procedures.
  • Complete all documentation and reporting as required.
  • Coordinate medication ordering with individuals, providers and pharmacies.
  • Observe, document and report signs of medication related side effects.
  • Communicate and coordinate with nursing and program staff.

Requirements: 

  • Bachelor’s degree in psychology, social work or related field.
  • Minimum of one year’s experience with developmentally disabled adults preferred.
  • MAP, CPR and First Aid certification preferred. 
  • Participate in on-call rotations, after hours and weekends.
  • Ability to communicate well in all formats, including with people with significant cognitive disabilities; familiarity with DDS regulations and with physical, behavioral and mental health issues for people with developmental disabilities.
  • Ability to work independently without a supervisor or other colleagues present.
  • At JRI, you will learn to provide physical interventions and verbal de-escalations to provide the safest therapeutic atmosphere for our clients! 
  • Must have valid driver’s license and ability to drive individuals in program vehicles and own vehicle if needed.
  • Background and driving record checks will be performed.
