As we continue to expand our care management initiatives, we are seeking a dedicated Medical Assistant to support our Remote Patient Monitoring (RPM) Program.
This is an excellent opportunity for a compassionate, organized, and technology-savvy Medical Assistant who is passionate about improving patient outcomes through proactive care coordination and remote monitoring.
Responsibilities
Support patients enrolled in the Remote Patient Monitoring (RPM) program.
Educate patients on the use of remote monitoring devices and ensure proper setup.
Monitor incoming patient health data and escalate abnormal findings to the clinical team.
Conduct outbound calls to engage patients, provide follow-up, and encourage compliance with monitoring protocols.
Document patient interactions accurately in the electronic medical record (EMR).
Coordinate with providers, nurses, and care management staff to ensure timely patient care.
Assist with scheduling follow-up appointments and addressing patient questions.
Maintain compliance with HIPAA and all organizational policies and procedures.
Qualifications
High school diploma or equivalent required; Medical Assistant certification is required.
Minimum of 1 year of experience as a Medical Assistant, preferably in primary care, care management, or chronic disease management.
Experience with Electronic Medical Records (EMR).
Excellent communication, interpersonal, and customer service skills.
Strong organizational skills with exceptional attention to detail.
Comfortable using technology and educating patients on medical devices.
Bilingual (English/Spanish) is required.
