    Job Description

    Primary Purpose:

    The person in this position is a member of the Patient Centered Health Care Home team where each team member plays an integral role in providing patient-centered health care. S/he is responsible in assisting the provider and the team in the overall coordination, preparation, examination and treatment of the patient. S/he will perform the minimum competency requirement for a medical assistant.

    Essential Duties:

    oReviews patient records prior to the visit

    oReminds patient of appointments as necessary

    oIdentifies if all previous orders were completed

    oEnsures that all necessary reports are in place

    oReviews gaps in care as necessary

    oPerforms check in duties appropriately and timely

    oRecords vital signs and other measurements accurately

    oUpdates all information related to meaningful use and PCHCH requirements

    oCarries out necessary check in clinical procedures appropriately per protocols

    oPerforms check out duties appropriately and timely

    oCarries out clinical procedures and order as directed by the provider efficiently and accurately

    oAdministers medication as ordered by the provider

    oAssists in simple wound dressing as necessary

    oRefers patients as necessary

    oComplete laboratory orders accurately

    oPerforms administrative duties

    oPrepares patient exam room

    oInventories supplies

    oCompletes necessary forms

    oFollows up on no-shows and schedule as necessary

    oSterilizes, labels and packs instruments as necessary

    oRetrieves and returns calls from patients, staff and other agencies in the community

    oCalls patients for laboratory and/or other reports

    oSchedules appointments as necessary

    • Operates basic medical equipment
    • Documents consistently and appropriately in EMR
    • Communicates with the provider and other members of the team to enhance patient care

    Regular and reliable on-site attendance is required.

    Other Duties:

    Performs other related duties as assigned

    Attends and participates in PCMH huddles, in-services education programs and meetings as required.

    Demonstrates excellent customer services to patients and/or providers and promotes positive image of KPHC.

    Provides feedback for quality improvement activities

    Arrives to work promptly. Inform supervisor when late or absent is imminent.

    Answers phones within reasonable amount of time and take accurate messages.

    Be open to feedback during evaluations and other times. Demonstrate willingness to improve job performance as necessary.

    Maintains and improves personal competence.

    Observes safety and security procedures; determine appropriate actions beyond guidelines; report potentially unsafe conditions; use equipment and materials properly

    Minimum Qualification Requirements:

    Skills/Knowledge:

    Ability to add, subtracts, multiply, and divides in all units of measure, using whole numbers, common fractions, and decimals.

    Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.

    Ability to apply common sense understanding to carry out instructions furnished in written, oral, or diagram form.

    Ability to deal with problems involving several concrete variables in standardized situations.

    Education:

    High School Diploma/GED required

    Medical Assistant Certification required

    One-year certificate from college or technical school; or equivalent combination of education and experience required.

    Current BCLS/CPR

    Experience:

    Previous experience with basic computer systems required

    Knowledge of basic medical terminology required.

    Bi-lingual preferred
