JOB SUMMARY: The medical assistant will provide clients with health care navigation in the community within the CCBHC/BHH/Virtual exam room model. The medical assistant will provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals. The medical assistant will work closely with medical providers, primary care teams, and social services agencies to provide short term care coordination and connection to resources for clients in efforts to improve their health and general well-being through education and provision of coordination of care and services.
