Family Health Care is currently seeking qualified candidates for our Medicare Annual Wellness Visit LPN position!

General Function: The Medicare Annual Wellness Visit (AWV) LPN supports Baldwin Family Health Care's (BFHC) Medicare population by coordinating and completing Medicare Annual Wellness Visits (AWVs) at assigned clinic sites. The LPN conducts scheduled face-to-face individual Medicare Annual Wellness Visits in the clinic setting and, when approved by the Chief Medical Officer, via telephone. The LPN completes all required documentation, assessments, and clinical workflows in accordance with established BFHC policies, procedures, protocols, standing orders, and CMS guidelines. Performs all responsibilities according to best practices in the field and reflecting the mission and vision of Baldwin Family Health Care.

Primary Responsibilities:

Complete Medicare Annual Wellness Visits

Scheduling and Documentation Health Assessments3 Patient Education Care Coordination Findings Review & Follow-Up Quality, Compliance & Productivity

Other Supporting Duties for Primary Care Providers and Medical Office: provides additional clinical and operational support to primary care providers and the medical office as requested by supervision including staff education, patient triage, participation with clinic emergency events, and adherence to workplace safety and infection control protocols. Supports the team in maintaining safe, compliant, and high-quality care delivery.

Formal Education Requirements: Required Qualifications:

Current Michigan LPN license with at least two years of experience with adult medicine and pediatric patients in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting, within the past five years. Graduation from an accredited nursing program.
