Medicare Skilled Scheduling Coordinator – Home Health

Activa Home Health is seeking an experienced Medicare Skilled Scheduling Coordinator with strong knowledge of OASIS workflows, Medicare home health operations, and Kinnser/WellSky. This role is ideal for someone who thrives in a fast-paced skilled home health environment and can confidently coordinate schedules for RNs, PTs, OTs, STs, HHAs, and other clinical staff while maintaining compliance, productivity, and continuity of care.

Key Responsibilities
- Manage and coordinate skilled home health schedules for clinical staff
- Support timely SOCs, ROCs, Recerts, Follow-Ups, and Discharges
- Monitor OASIS-related scheduling timelines and workflow compliance
- Pull and manage reports including productivity, missed visits, unsigned orders, and overdue documentation
- Utilize Kinnser/WellSky daily for scheduling, workflow management, and communication
- Collaborate closely with Clinical Managers, Intake, Nursing, and Therapy teams
- Resolve scheduling conflicts and support urgent patient care needs
- Ensure continuity of care and timely visit completion 
- Maintain accurate scheduling documentation within the EMR system 

Qualifications
- Minimum 2 years of Medicare skilled home health scheduling experience 
- Strong knowledge of Medicare home health and OASIS processes required
- Proficiency in Kinnser/WellSky required
- Experience coordinating RN, LPN, PT, OT, SLP, RRT, PTAs, OTAs and interdisciplinary clinician schedules
- Strong organizational, communication, and multitasking skills
- Ability to work independently in a high-volume environment
- Intake Coordinator Experience preferred

Work Environment and Physical Requirements 
This position operates primarily in an office setting with standard computer and phone work. You should be comfortable sitting for extended periods, managing detailed data, and occasionally lifting up to 15 pounds. Reasonable accommodations will be provided as needed. This position involves no exposure to blood or bodily fluids and no direct patient care responsibilities. 

This position is based in our Stuart, Florida office and requires successful completion of a Florida Level 2 background screening.
