Position Summary
As a Mental Health Counselor at Community Health Center of Buffalo, Inc., you’ll provide outpatient clinical services in an Article 31 setting, supporting individuals navigating mental health and co-occurring challenges.
This role combines meaningful, relationship-based clinical work with a strong interdisciplinary team environment. You’ll play a key role in helping patients access care, build stability, and move forward in their recovery using trauma-informed, person-centered approaches.
What You’ll Do
· Conduct comprehensive psychosocial and diagnostic assessments, including risk evaluations
· Develop and implement individualized, recovery-oriented treatment plans
· Provide individual therapy using evidence-based approaches such as CBT, DBT, and trauma-informed care
· Deliver crisis intervention and coordinate with higher levels of care when needed
· Collaborate with medical providers, care teams, and community partners to support whole-person care
· Participate in team meetings, case conferences, and clinical supervision
· Connect patients to community resources including housing, substance use treatment, and social services
· Maintain timely and compliant clinical documentation in accordance with OMH and Article 31 standards
· Meet productivity expectations while maintaining quality, patient-centered care
What We’re Looking For -Qualifications
· LMHC, LMSW, or LCSW (NYS permit holders encouraged to apply)
· 3+ years of clinical experience preferred
· Experience with trauma-informed care; EMDR experience is a plus
· Strong interpersonal, communication, and organizational skills
· Comfort working in a fast-paced, team-based environment
Schedule & Physical Requirements
· Combination of sitting, standing, and movement throughout the day
· Some evening or weekend availability may be required
Compensation
· $52,000–$58,000 annually
