Qualifications:
Education: Master's degree in Nursing from an accredited program.
Licensure: Must have an active New York State license. Must be board-certified or board-eligible.
Experience: At least 1 year of experience as a Nurse Practitioner (not necessarily in Nephrology). Nephrology experience is a plus but not required.
Technical Skills: Proficient in patient assessment, diagnostic test interpretation, medication management, and electronic health records. Willingness to learn nephrology-specific procedures and treatments.
Soft Skills: Strong clinical judgment, excellent communication and interpersonal skills, empathy, attention to detail, and the ability to work collaboratively within a multidisciplinary team.
Key Responsibilities:
Conduct comprehensive patient assessments, including medical histories, physical examinations, and renal function evaluations.
Assist in the diagnosis and management of kidney-related conditions such as chronic kidney disease, hypertension, electrolyte imbalances, glomerulonephritis, and end-stage renal disease.
Order, interpret, and analyze diagnostic tests including blood work (creatinine, BUN, GFR), urinalysis, and imaging studies.
Develop and implement individualized treatment plans in collaboration with the supervising nephrologist.
Prescribe medications, including antihypertensives, diuretics, and phosphate binders, as appropriate.
Educate patients and families on kidney disease management, dietary modifications (fluid, sodium, potassium, phosphorus restrictions), medication adherence, and when to seek emergency care.
Coordinate care with dialysis centers, transplant teams, and other specialists within the multi-specialty practice.
Monitor patient progress through regular follow-up visits and adjust treatment plans as needed.
Maintain accurate, thorough, and timely documentation in electronic medical records.
Stay current with advancements in nephrology through continuing medical education and collaboration with supervising physicians.
