Kidney disease is rarely simple, and patients feel every delay. This role exists to keep complex nephrology care moving—clinic, dialysis, and inpatient—so people leave with a clear plan, not another unanswered question. You will practice at the top of your license beside nephrologists who expect judgment, not task completion.
Performance Objectives
Manage a defined panel of CKD, dialysis, transplant-follow-up, and electrolyte patients so visit plans, labs, and medication changes are complete before the patient leaves.
See new and return patients independently, presenting only the decisions that need physician input, and close same-day notes that support billing and continuity.
Round on assigned inpatients or dialysis shifts, stabilize acute issues, and hand off a prioritized plan that the next clinician can act on without a callback.
Cut avoidable readmissions and missed dialysis complications by escalating fluid, access, and infection problems early and documenting the trigger and response.
Own results follow-up: abnormal labs, imaging, and consults are reviewed, communicated, and acted on within the practice standard, with no orphaned orders.
Teach patients and families the why behind diet, meds, access care, and transplant or dialysis choices so adherence and informed consent improve, not just documentation.
Partner with nurses, dietitians, social work, and dialysis units to remove the bottlenecks you see—scheduling, prior auth, or handoff gaps—and measure whether they stay fixed.
Environment & Resources
You report to the lead nephrologist and work inside a physician-APP team covering clinic, hospital, and dialysis.
Support includes experienced MAs, RNs, a dietitian, social work, and a dedicated scheduler—not a solo inbox.
Charting is in a shared EHR with order sets, remote access, and protected admin time built into the template.
You will have collaborating-physician backup, a clear escalation path, and CME support aimed at nephrology, not generic credits.
Essential Qualifications
Graduate of an accredited PA program and NCCPA certification, with an active state license and DEA where prescribing is required.
At least two years as a PA in nephrology, inpatient medicine, or dialysis—or a structured nephrology fellowship—with independent management of CKD and acute kidney injury.
Demonstrated ability to run a full clinic or rounding list, write complete notes the same day, and adjust antihypertensives, diuretics, binders, and anemia therapy within protocol.
Fluency with dialysis access problems, electrolyte emergencies, and when to stop, start, or change renal replacement therapy.
Clear, direct communication with patients who are scared, tired, or non-English speaking, plus reliable collaboration with dialysis staff.
Comfort with EHR orders, results tracking, and quality metrics; no pattern of incomplete follow-up.
Why This Role
Real scope: you manage disease, not just room patients for someone else.
Mixed setting—clinic, hospital, and dialysis—so skills stay sharp and days do not blur into one template.
Physician partners who teach and back you, with a schedule that protects documentation time.
Patients you follow over years, including the hard conversations about dialysis and transplant.
A team measured on outcomes and access, not volume alone.
If you want ownership of kidney care and can show the outcomes above, apply with a brief note on the panel you have managed and the decision you are proudest of. We read every application.
