(CFHC) is a non-profit 501(c)3 Federally Qualified Health Center (FQHC) and designated as a medically underserved “safety net” provider for citizens residing in the tri-county area of Hardee, Highlands, and Polk Counties. Founded in 1972 in response to critical gaps in health care within the agricultural community, CFHC opened its first clinic with a doctor and a nurse and began serving farm workers in the Frostproof community. Today, CFHC has 15 community health centers throughout the tri-county area providing comprehensive health care programs such as adult and pediatric primary care, behavioral health, adult and pediatric dental care, HIV and Hepatitis B/C, nutritional counselling, obstetrics and gynecology, optometry, pharmacy, labs, x-rays and a wide range of social services for our patients. POSITION SUMMARY: Provide out-patient comprehensive medical services based on respective specialty including preventive and health maintenance services, acute and chronic care, clinical evaluation, diagnosis, and consultation services to CFHC clinical staff and outside providers who see CFHC patients. In the event hospitalization is required, will refer to hospitalist at local hospital and will monitor care upon discharge or for some areas of specialty, in-patient duties and hospital rounding may be required.
