Salary: $100,000 to $110,000 annually
Position Summary: The Hospice Aide Manager position serves as a leader, providing supervision for direct care staff. This position services as a management position with management responsibility for their team and serves as a liaison between the Director of Patient Care, and the Hospice Aides on their respective team, patients and their families, and the community. This position will work closely with the leadership team on the implementation of policies and procedures while carrying out the organization’s purposes in a manner consistent with the culture, goals and established practices.

Qualifications:
Registered Nurse (RN) with a current and active state nursing license.
Minimum of 3 years of clinical nursing experience, preferably in hospice, long term care, community health or palliative care settings.
Must have the ability to direct and supervise staff and lead in a professional manner
Current Minnesota Driver’s License, insurance and daily access to reliable vehicle
Excellent written and verbal communications skills
Excellent with relationship development internally and externally
Ability to think critically
Ability to network within the referral communities
Strong knowledge of hospice regulations including CMS, state licensure and conditions of participation
Ability to respect confidentiality
Job Duties:
Supervise CNAs
Collaborate with all departments
Assist in the recruiting, selecting, hiring and orienting of qualified individuals
Manage caseloads and territories of Hospice Aides
Supervisor on-call responsibilities
Support team through development of programs, goals and objectives consistent with those of the organization’s overall strategic plan and initiatives
Actively participates in leadership meetings, collaborating and communicating with leadership team
Assist and/or lead new projects as assigned
Facilitate a cohesive team to provide excellent end of life care
Participate in IDG meetings
Responsible for building and maintaining employee morale, motivating employees to reach their full potential including professional development
Uses available resources and data to guide excellent patient care
Assist in budget process and staffing plans
Support and participate in quality improvement
Identify and act upon improvement opportunities for the overall success of Grace Hospice
Assist in overseeing compliance of clinical charts
