Currently serving over 1,500 patients in two different states and looking to grow to over 10,000 patients in 5 states, over the next 3-5 years. Its focus is advocating and implementing integrated care model approach to underserved population undergoing behavioral health related issues.
What You'll Do
The Nurse Practitioner (NP) provides advanced clinical care to individuals receiving services in PHRI’s substance use and behavioral health programs. The NP conducts evaluations, diagnoses, medication management, and ongoing care coordination under approved protocols and in collaboration with the medical and clinical team.
What You'll Bring
Conduct comprehensive physical and psychiatric assessments to identify medical and behavioral health conditions.
Diagnose client conditions and collaborate in developing individualized treatment plans that incorporate behavioral, physical, and social determinants of health.
Prescribe psychotropic and general medications; monitor client progress and manage side effects or treatment adjustments as necessary.
Provide crisis intervention and participate in safety planning with the interdisciplinary team.
Review and authorize medication orders in accordance with federal and state law.
Consult with physicians, therapists, case managers, and other healthcare professionals to ensure coordinated care.
Educate clients and families on medication use, treatment plans, wellness strategies, and health maintenance.
Ensure accurate documentation in the Electronic Medical Record (EMR) system and maintain up-to-date progress notes.
Participate in case conferences, audits, and interdisciplinary team meetings.
Ensure compliance with all federal, state, and local regulations including HIPAA, OSHA, and 42 CFR Part 2.
Contribute to the development and review of clinical policies and medical protocols.
Provide supervision, mentorship, or guidance to nursing and support staff as assigned.
Participate in quality assurance and performance improvement projects.
Maintain current licensure, certifications, and required continuing education.
Perform other duties as assigned.
Required Qualification
Current and unencumbered Nurse Practitioner licensure in Washington, D.C.
Board Certification in Family Nurse Practitioner (FNP) or Psychiatric-Mental Health Nurse Practitioner (PMHNP) required.
Master’s or Doctorate degree in Nursing from an accredited academic institution.
Minimum of 3 years of clinical experience in behavioral health or substance use disorder treatment settings.
Strong knowledge of trauma-informed care principles, psychopharmacology, and co-occurring disorders.
Experience with EMR systems (e.g., Credible, GAINS, LOCUS, or ICAMS) highly preferred.
Current CPR/BLS certification.
Demonstrated ability to work effectively in interdisciplinary settings and with diverse populations.
Proficient computer skills in Microsoft Outlook and other standard software applications.
Legally authorized to work in the U.S.A.
