Nurse Practitioner - Certified Wound Care (Clinic-based)

Education: Master's Degree

Master's degree in Nursing from an accredited nursing program. Licensed as a Registered Nurse by the State Board of Nursing (SBON) in the state(s) where the nurse practices. Board certified and licensed as a Nurse Practitioner in the state(s) where the Nurse Practitioner practices.

Specialty Education/Certification:

Preferred: Completion of a WOCN Society-accredited educational program and current certification by the WOCNCB in wound care, including one of the following:

• CWCN - Certified Wound Care Nurse
• CWON - Certified Wound Ostomy Nurse
• CWOCN - Certified Wound Ostomy Continence Nurse

Alternative:

• WCC - Wound Care Certified through the National Alliance of Wound Care and Ostomy
• CWS - Certified Wound Specialist through the American Board of Wound Management

Minimum Experience:

Nurse Practitioner: Minimum of one (1) year of experience in a wound-related Nurse Practitioner role with one of the following

• Three (3) years of experience as a CWCN, CWON, or CWOCN
• Five (5) years of experience as a WCC or CWS

Roles and Responsibilities:

• Provide in-person evaluation and management visits for patients with acute and chronic wounds and related skin disorders within a defined geographic area traveling to different clinic locations throughout the week
• Perform medically necessary debridement and procedures within the Nurse Practitioner's scope of practice and applicable CMS, payer, state, and federal guidelines to promote healing and prevent complications.
• Estimated time spent providing in-person care is approximately 80%, although this may vary based on patient and program needs.
• Work with a Medical Assistant when applicable and delegate appropriate tasks to the Medical Assistant consistent with the MA's training, competency, role, and applicable laws and regulations.
• Provide appropriate clinical direction to the Medical Assistant while maintaining responsibility for clinical assessment, diagnosis, treatment decisions, orders, procedures, and other functions requiring the Nurse Practitioner's professional judgment.
• Utilize the Medical Assistant to support efficient patient care, including appropriate clinical support, wound photography, dressing-related tasks, supplies, patient preparation, and other delegated responsibilities.
• In-person productivity for clinic visits averages six (6) per day or thirty (30) visits per week subject to patient acuity, clinic scheduling, program requirements, administrative responsibilities and other operational factors.
• Productivity expectations are based on actual working days and may be adjusted for PTO, holidays, sick time, orientation, training, meetings, administrative responsibilities, or other company-approved activities.
• Flexibility is expected to increase productivity during periods of high demand, including weeks preceding or following planned PTO or other anticipated staffing needs.
• Provide evaluation and management visits via telehealth, virtual care, or digital monitoring modalities for patients with acute and chronic wounds and related skin disorders. Estimated time spent providing virtual care is approximately 20% in addition to the applicable in-person productivity standard.
• Support the company overall by completing patient assessments during periods of high demand for virtual/telehealth assessments in other geographic areas and within states in which the provider is licensed.
• Obtain additional state licensure when requested by The Wound Company and when reasonably necessary to support company programs. The Wound Company will cover approved licensing expenses.
• Follow appropriate infection prevention protocols and procedures for the handling of contaminated equipment, dressings, bodily fluids, and other potentially infectious materials.
• Document services accurately and appropriately to support the level of billing and reimbursement consistent with CMS, payer, and company guidelines.
• Document patient care in real time whenever reasonably possible and clearly and concisely in the appropriate EMR for enrolled patients.
• Complete all billable and non-billable service documentation within one (1) business day.
• Complete required orders, care plans, follow-up documentation, and other clinical documentation within established company timeframes.
• Use judgment and critical-thinking skills to assess, diagnose, and identify appropriate outcomes and interventions to promote safe patient engagement and positive clinical outcomes.
• Use evidence-based assessment techniques, tools, clinical guidelines, and available data and information to identify problems and needs of patients with acute and chronic wounds and other related skin disorders.
• Collect pertinent biological, physical, functional, psychosocial, and environmental data using a systematic process to identify patient needs.
• Prescribe care within the provider's scope of practice, including treatments, medications, supplies, and other appropriate interventions.
• Collaborate with virtual and field-based clinical specialists to coordinate care and delegate appropriate pre-visit and follow-up activities based on patient needs.
• Facilitate and coordinate care with other providers, including home health, primary care, specialists, skilled nursing facilities, assisted living facilities, caregivers, and other members of the patient's healthcare team, through the EMR and approved digital communication modalities to enhance care and prevent and manage complications.
• Communicate respectfully, clearly, professionally, and concisely with patients, caregivers, providers, facilities, and other team members.
• Evaluate processes, policies, procedures, and clinical protocols/guidelines for the care of patients with acute and chronic wounds and other related skin disorders with other team members as requested.
• Recommend revisions to clinical processes, policies, procedures, and protocols when warranted by evidence, regulatory requirements, clinical practice, or operational needs.
• Participate in the development of the Nurse Practitioner program, including review and improvement of processes supporting billing, documentation, patient outcomes, and quality measures.
• Provide clinical input in the design and implementation of quality-improvement initiatives to optimize outcomes of care for patients.
• Participate in collaborative meetings with team members to support existing and new programs.
• Maintain current wound-care certification and Nurse Practitioner licensure/certification and participate in ongoing educational opportunities to maintain clinical expertise and remain current with evidence-based wound-care practices.
• Maintain current Registered Nurse licensure and Nurse Practitioner national board certification as applicable to the provider's practice.
• Maintain current BLS certification.
• Other duties as assigned.
• Uphold company policies for HIPAA compliance to maintain patient privacy, confidentiality, and data security.
• Maintain access to high-speed internet at home at a level necessary to complete virtual clinical responsibilities.
• Maintain company equipment and supplies in a clean, secure, and working condition.
• Maintain a cellular phone plan sufficient for required work-related communication.
• Maintain appropriate document destruction measures for the home office.
• Have reliable means of transportation and maintain automobile insurance meeting company requirements for onsite and in-person assessments or utilize and maintain company provisioned vehicles when applicable.
• Maintain the ability to travel throughout the assigned geographic area to provide patient care.
