Nurse Practitioner Home Health

This is a Home Health Care Service company that provides care to people with Medicare and is offering a $7,500 sign-on bonus. They are on a mission to transform the Medicare experience for everyone. As the lead nurse practitioner, you will influence and ensure the best clinical care for your patients.

Pay:

Starting at $115,000 Based on experience

$7,500 Sign-on Bonus

Requirements:

• Masters' degree or higher

• Michigan State Nurse Practitioner License

• 1 year of home health experience

• Value-based care experience (preferred)

• Experience working in a start-up (preferred)

• 40 Hours Per week

Job Duties:

• Lots of driving, In-home house calls (80% of the time)

• Comprehensive management of patient conditions, including assessment, diagnosis, treatment, and consultation, on a longitudinal basis.

• Transitions of care support via collaboration with post-acute care facilities.

• Accurately document patient conditions and encounters in an EMR.

• Ensure high levels of quality and patient satisfaction with each encounter.

• Partner with the care team to identify and eliminate social determinants of healthcare for your patients, including functional capacity, home environment, health literacy, housing instability, food insecurity, and caregiver support.
