Job Description
What You’ll Do:
- Perform face-to-face visits in accordance with Medicare requirements to support hospice eligibility and recertification
- Conduct patient assessments, intake visits, and follow-up care in the home setting
- Collaborate with the Hospice Medical Director, physicians, and interdisciplinary team to develop and manage patient care plans
- Provide symptom management, prescribe medications within scope, and support overall patient comfort
- Communicate clinical findings and changes in patient condition to the care team to ensure continuity of care
- Educate patients and families on disease progression, care options, and hospice philosophy
- Participate in interdisciplinary team meetings and contribute to care planning discussions
- Coordinate care across settings, including home, hospital, and long-term care facilities
- Facilitate family meetings and provide guidance on goals of care and treatment decisions
- Ensure timely, accurate documentation and billing in the electronic medical record
- Support staff education and provide training on hospice and palliative care best practices
- Coordinate resources including equipment, medications, and supportive services such as social work and chaplaincy
What We’re Looking For:
- Clinical Expertise – Strong assessment, diagnostic, and patient management skills in hospice or palliative care
- Experience – At least 2 years as a Nurse Practitioner in acute care, home health, geriatrics, family practice, or palliative care
- Compassion & Emotional Intelligence – Ability to support terminally ill patients and families with empathy and professionalism
- Collaboration & Communication – Works effectively with physicians, interdisciplinary teams, and community providers
- Independent & Organized – Able to manage time, prioritize patient needs, and work autonomously in the field
- Technology Skills – Proficient with EMR systems and standard software applications
- Commitment to Hospice Philosophy – Demonstrates understanding of end-of-life care, disease progression, and patient-centered decision making
Qualifications:
- Current, unrestricted Virginia Nurse Practitioner license
- Minimum of 2 years of Nurse Practitioner experience in a relevant clinical setting
- Hospice and palliative care certification preferred
- Strong knowledge of disease progression and end-of-life care
- Proficiency with electronic medical records and documentation systems
- Valid Virginia driver’s license and reliable transportation
- Ability to travel to patient homes and care settings as needed
