Now Hiring - Advanced Practice Nurse/Internal Medicine/General Practice NPs

Salary Range: $125,000 to $135,000 a year
The Advanced Practice Nurse will be responsible for, but are not limited to:
- Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
- Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
- Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
- Consult with other members of interdisciplinary team to contribute to the patients plan of care.
- Prepare and maintain accurate patient records, charts and documents.
- Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
- Ensure collaboration with patients, caregivers, medical directors and members of IDT.
Education:
- Masters Degree in Nursing
Professional Experience:
- Must have a Nurse Practitioners license in the state of operations
- Must have, at minimum, ONE year experience in the capacity as a Nurse Practitioner in a hospital, skilled nursing or acute care setting (or similar field)
- Board Certified in Internal Medicine, or General Practice
- Must have a Masters Degree
- Must have a valid drivers license and a reliable source of transportation
- Must be willing to travel locally
