Direct Hire Opportunity!

Position Overview

Our client is an innovative, rapidly growing healthcare organization seeking an experienced Nurse Practitioner or Physician Assistant with a strong acute care background to join its collaborative clinical team.

This opportunity is ideal for providers experienced in acute care, hospital medicine, internal medicine, or managing medically complex patients who want to apply their clinical expertise in a patient-centered outpatient environment.

Our client specializes in delivering comprehensive healthcare to Medicare-aged and medically underserved populations. Their value-based care model prioritizes quality outcomes, early intervention, chronic disease management, and reducing unnecessary hospitalizations rather than patient volume.

Key Responsibilities

  • Evaluate, diagnose, and treat acute illnesses and complex chronic conditions.
  • Perform comprehensive assessments, physical examinations, and diagnostic evaluations.
  • Manage patients with multiple comorbidities, including CHF, COPD, diabetes, and cardiovascular conditions.
  • Recognize clinical deterioration and determine appropriate interventions or escalation of care.
  • Order and interpret diagnostic testing, laboratory results, and imaging.
  • Develop individualized treatment plans and manage medications.
  • Coordinate post-hospitalization care to reduce readmissions and improve patient outcomes.
  • Collaborate with physicians, nurses, specialists, and interdisciplinary care teams.
  • Educate patients and families regarding treatment plans and disease management.
  • Maintain accurate documentation using electronic health records.

Compensation & Benefits

  • Competitive base salary plus quality-based bonus opportunities.
  • No RVU or patient-volume-based compensation.
  • Full malpractice coverage, including tail.
  • Comprehensive health insurance and retirement benefits.
  • Generous paid time off and CME support.
  • Modern, state-of-the-art outpatient facilities.
  • Advanced electronic health record technology.
  • Collaborative clinical environment with dedicated physician and nursing support.
  • Opportunity to provide comprehensive care without the demands of a traditional inpatient schedule.

Qualifications

  • Graduate of an accredited Nurse Practitioner or Physician Assistant program.
  • Active, unrestricted state license and appropriate national certification.
  • Current BLS and ACLS certifications.
  • Acute care, hospitalist, emergency medicine, or complex internal medicine experience strongly preferred.
  • Experience managing medically complex patients with multiple acute and chronic conditions.
  • Strong clinical assessment, diagnostic reasoning, and medical decision-making skills.
  • Experience treating geriatric or Medicare-aged populations preferred.
  • Ability to recognize changes in patient condition and coordinate appropriate interventions.
  • Excellent communication and interdisciplinary collaboration skills.
  • Passion for delivering high-quality, patient-centered healthcare.
  • Willingness to travel between nearby clinical centers as needed.

Ideal Candidate

The ideal candidate is a clinically confident NP or PA with experience in acute care, hospital medicine, emergency medicine, or complex chronic disease management.

This opportunity is particularly attractive to providers who enjoy managing challenging medical conditions but are seeking a more predictable outpatient environment with greater continuity of care.

Why Join Our Client?

Bring your acute care expertise into a setting where quality matters more than quantity. Enjoy a collaborative clinical team, competitive compensation, strong organizational support, and the opportunity to make a meaningful difference in the lives of older adults.

Use your clinical expertise to improve outcomes, prevent unnecessary hospitalizations, and deliver the care patients deserve.
