Nephrology Nurse Practitioner (NP)Fox Valley Medical Associates is hiring a full-time Nurse Practitioner–Nephrology (Onsite) to extend physician coverage and strengthen continuity of care across dialysis clinics, hospitals, and limited outpatient nephrology settings.Key responsibilities include: Round on dialysis patients and monitor volume status, blood pressure, labs, dialysis tolerance, medications, anemia management, CKD-MBD, and access concerns. (Aurora 3 outpatient dialysis clinics, Batavia 2 outpatient dialysis clinics, Yorkville 1 outpatient dialysis clinic)Provide inpatient nephrology support at Northwestern Medicine Delnor Hospital and Rush Copley Medical Center for stable ESRD and AKI follow-up.Coordinate care among nephrologists, dialysis staff, hospital teams, case management, dietitians, social workers, and nursing teams.Assist with discharge planning, renal medication review, patient education, and timely outpatient follow-up.Ordering and interpreting labs/imaging; prescribing medications (including controlled substances within scope); Provide first-line hospital nephrology coverage every other weekend, with physician backup available for escalation.Document encounters accurately and support quality, safety, and care-continuity initiatives.Required qualifications/skills:Current Nurse Practitioner licensure in the State of Illinois.Current APRN certification.Graduate of an accredited Nurse Practitioner program.Current DEA registration or ability to obtain as appropriate to the position.Current BLS certification.Ability to obtain privileges and credentialing at Northwestern Medicine Delnor and Rush Copley.Ability to obtain credentialing with applicable dialysis organizations and payers.Ability and willingness to travel regularly between hospitals, dialysis facilities, and outpatient locations.Valid driver's license and reliable transportation.Strong clinical judgment and ability to recognize changes in patient acuity.Strong written and verbal communication skills.PreferredPrior nephrology experience.Dialysis experience.Hospital rounding experience.Familiarity with hemodialysis and peritoneal dialysis.Experience with acute kidney injury and inpatient nephrology.Experience working collaboratively with physician specialists.If you’re ready to deliver safe, patient-centered nephrology care across settings, apply today.Your BenefitsMedical, Dental, Disability and Vision coverage401(k) w/matchLife InsuranceCompetitive salary ($120,000–$130,000)Paid Time OffA Typical ShiftYour Nurse Practitioner–Nephrology schedule follows a two-week rotation. Some weekdays start with dialysis facility rounding, while others begin at the hospital for inpatient follow-ups; you may also have a limited outpatient session depending on patient needs. You’ll move between Aurora, Batavia, Geneva and Yorkville, staying organized as the census changes. Every other weekend includes Saturday/Sunday hospital coverage, followed by a planned Monday off to reset before the next stretch.
