About the Role Were seeking a Nurse Practitioner to join a collaborative primary care team in a full-time outpatient setting. This role offers the opportunity to provide comprehensive care for patients of all ages, with a focus on quality, continuity, and patient-centered treatment.
What Youll Do
Perform patient histories, physical exams, and preventive screenings
Diagnose and manage acute and chronic conditions across diverse populations
Order, interpret, and follow up on diagnostic tests
Collaborate closely with physicians, nurses, and specialists to coordinate care
Document all patient interactions accurately in the EMR
Participate in quality improvement initiatives and team-based care planning
Help connect patients to community health and wellness resources
What Youll Need
Active and unrestricted Nurse Practitioner license
Current national board certification and DEA registration
CPR certification
Experience in outpatient or community health settings preferred
Strong clinical judgment and the ability to work independently and as part of a team
