(CHCS), a Federally Qualified Health Center (FQHC), is seeking a compassionate and dedicated Nurse Practitioner (NP) or Physician Assistant (PA) to join our team serving patients at our Wrightsville Clinic and Johnson County School Clinic.This is an excellent opportunity for a provider who enjoys outpatient primary care and is passionate about improving access to quality healthcare in rural communities.Position ResponsibilitiesThe NP/PA will provide comprehensive primary care services to patients across the lifespan and will divide time between our Wrightsville Clinic and Johnson County School Clinic. Responsibilities include:Conducting patient examinations, assessments, and evaluationsDiagnosing and treating acute and chronic health conditionsOrdering, reviewing, and interpreting appropriate laboratory and diagnostic testingPrescribing medications and developing individualized treatment plansProviding preventive care, health education, and wellness counselingManaging chronic conditions and coordinating care with other healthcare professionalsProviding age-appropriate care to students in the school-based health centerMaintaining accurate and timely documentation in the electronic medical recordCollaborating with physicians, clinical staff, school personnel, and other members of the care teamFollowing CHCS clinical, quality, compliance, and patient-care policies and proceduresQualificationsCurrent Georgia licensure as a Nurse Practitioner or Physician AssistantCurrent national certification, as applicableCurrent DEA registration or eligibility to obtain, if required for the positionCurrent BLS certificationPrimary care, family medicine, pediatric, or community health experience preferredStrong communication, organizational, and patient-care skillsAbility to work effectively with patients of diverse ages and backgroundsCommitment to providing compassionate, patient-centered careWhy Join CHCS?Community Health Care Systems, Inc. is committed to providing accessible, high-quality healthcare to the communities we serve.
