    Job Title:

    Nurse Practitioner General Medicine (SNF/ALF Setting) Location: Paducah, KY and surrounding area (within 3540 minutes) Schedule: Minimum 2 days/week (flexible schedule)

    Job Description:

    We are seeking an experienced Licensed Nurse Practitioner (NP) to provide internal medicine services in a Skilled Nursing Facility (SNF) and/or Assisted Living Facility (ALF) environment. This is an opportunity to step into a rewarding clinical leadership role as an on-site SNF-ist, delivering consistent care to geriatric residents in long- and short-term care settings.In this role, you will serve as the facilitys day-to-day provider, coordinating with nursing staff, medical directors, and attending physicians to oversee resident care. Your work will directly impact patient outcomes, reduce unnecessary hospitalizations, and improve quality of life for aging populations.

    Key Responsibilities:

    Provide direct patient care including health assessments, diagnosis, and treatment planningPerform monthly stability visits and acute condition evaluationsPrescribe necessary medications and treatments within state scope of practiceCommunicate with facility staff, physicians, specialists, and family membersDocument all care provided using appropriate CPT codes and clinical documentation systemsParticipate in advanced care planning and patient care conferencesCollaborate with interdisciplinary teams to deliver holistic care

    Requirements:

    Active Advanced Practice Registered Nurse (APRN) license in the state of KentuckyCurrent DEA licenseMinimum 1 year of experience in a Skilled Nursing or Long-Term Care setting (preferred)Excellent clinical assessment and documentation skillsStrong communication and collaboration abilitiesAbility to work independently and manage a flexible schedule

    Compensation & Schedule:

    Pay-per-encounter modelMinimum commitment of 2 days per week (you choose the days)Opportunity for additional days as desiredSingle location in the Paducah, KY area
