POSITION OVERVIEW

The Family Nurse Practitioner provides comprehensive primary care services by evaluating patients, diagnosing medical conditions, developing treatment plans, and educating patients and families on disease prevention and health management.

Working collaboratively with an interdisciplinary healthcare team, the FNP will play an essential role in delivering exceptional patient care while promoting positive health outcomes in a supportive clinical environment.

KEY RESPONSIBILITIES

• Conduct comprehensive patient assessments, examinations, and medical evaluations.
  
• Diagnose acute and chronic medical conditions and develop individualized treatment plans.
  
• Provide ongoing patient education regarding disease prevention, wellness, medications, and treatment plans.
  
• Collaborate closely with physicians, nurses, therapists, and other healthcare professionals to coordinate patient care.
  
• Maintain accurate and timely documentation within the electronic medical record (EMR/EHR) system.
  
• Monitor patient progress and adjust treatment plans as appropriate.
  
• Promote preventive healthcare and healthy lifestyle practices.
  
• Ensure compassionate, patient-centered care while maintaining the highest standards of clinical excellence.
  
• Participate in continuing education and professional development to remain current with evidence-based clinical practices.

REQUIRED QUALIFICATIONS

• Current, unrestricted New Mexico Family Nurse Practitioner (FNP) license.
  
• Minimum of three years of Family Nurse Practitioner experience.
  
• Proficiency with EMR/EHR systems such as Epic or athenahealth.
  
• Telehealth experience is highly desirable.
  
• Excellent verbal and written communication skills.
