Our client, a mission-driven provider transforming care for individuals with complex needs, is hiring a Nurse Practitioner – Palliative Care. In this hybrid role, the Nurse Practitioner delivers evidence-based, high-quality care to complex and seriously ill patients through telemedicine and in-person visits, with a focus on symptom management, timely hospice transitions, and improving quality of life while reducing unnecessary hospitalizations.

Responsibilities

  • Independently perform comprehensive palliative symptom management visits and provide plan-of-care oversight in accordance with state-specific law
  • Order and interpret diagnostic and therapeutic tests only as needed based on age-specific needs and a life expectancy of less than one year
  • Prescribe appropriate pharmacologic and non-pharmacologic treatment modalities
  • Conduct goals of care discussions, prognostication, and advance care planning documentation, including POLST forms
  • Complete transition-of-care visits following Emergency Room, hospital, or Skilled Nursing Facility discharge, including warm handoffs and frequent communication to reduce readmission risk
  • Facilitate transitions to hospice and other appropriate health care settings
  • Collaborate daily with the interdisciplinary team (IDT) to review urgent clinical issues, transitions of care, and high-risk patients
  • Serve as a clinical resource by providing education, anticipatory guidance, and connection to community and support resources
  • Participate in required meetings and trainings and join an after-hours on-call rotation
  • Demonstrate familiarity with Medicare coding requirements, including FFS CPT, ICD-10, HCC, ACP, Palliative Care, and CCM documentation standards

Requirements

  • ANP, AGNP, or FNP certification
  • Graduate of an accredited Nurse Practitioner program
  • DEA licensure, or ability to obtain ACHPN certification (client absorbs all associated fees)
  • Active, unrestricted New Jersey RN and NP license, with ability to obtain additional state licensure
  • Preferred: 2+ years providing NP care to complex, seriously ill patients (new-graduate NPs with hospice/palliative or home care experience will be considered)
  • Strong preference for active licensure in CA, OR, WA, ID, UT, NJ, HI, FL, TX, and GA
  • Able to work Monday through Friday, 8:30am–5:00pm Eastern Time, with rotating evening/weekend on-call coverage
  • Hybrid travel required across northern New Jersey, with telehealth coverage across NJ and other states where the client operates
