Nurse Practitioner/ Physician Assistant

Pride Health is hiring a Travel Nurse Practitioner - Long-term Care - to support our client's healthcare team in Albany, NY.

Schedule: Monday - Friday 8a-5p
Location: Rensselaer & Albany, NY
Pay: $69 hourly 
Contract Length: 53 Week/s
 
Key Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans.
• Manage acute and chronic medical and behavioral health conditions.
• Maintain accurate clinical documentation, including ICD-10 coding.
• Collaborate with interdisciplinary healthcare teams to coordinate patient care.
• Lead advanced illness and end-of-life care planning discussions.
• Educate and counsel patients, families, and caregivers on treatment plans and health management.

Provide care to patients with complex medical, social, behavioral, and functional needs.
 
Required Qualifications

• Active, unrestricted New York NP or PA license, or ability to obtain one by the start date.
• Graduate of an accredited MSN/DNP program for NPs or an accredited Physician Assistant program.
• Current national board certification:
  • NP: AANP or ANCC certification in an eligible specialty, including Family, Adult, Gerontology, or Adult-Gerontology Acute Care.
  • PA: NCCPA certification.
• Active DEA registration and prescriptive authority.
• Ability to obtain a collaborative practice agreement, if required by state law.
• Reliable transportation and ability to meet the physical demands of the position, including lifting up to 30 pounds and navigating stairs.
• Ability to meet applicable vaccination requirements and pass a pre-employment drug screening.
