About the Opportunity

Tenavi Staffing is recruiting a Nurse Practitioner or Physician Assistant for our healthcare client to care for older adults with chronic conditions and complex medical needs.

This position combines home visits, clinic-based care, and virtual assessments. You will work with physicians, advanced practice clinicians, case managers, and other healthcare professionals to assess patients, develop individualized care plans, and coordinate services that support their health and quality of life.

Patients are typically age 65 and older. Home and virtual visits generally last 45–60 minutes, depending on whether the appointment is an initial or annual assessment.

Responsibilities

Conduct comprehensive in-home assessments, medication reviews, and health screenings.

Provide clinic-based and virtual care as part of a Monday–Friday schedule.

Identify and assess conditions requiring care management or active medical treatment.

Educate patients about their health conditions, treatment options, and care plans while considering their goals and preferences.

Collaborate with medical leadership and the broader care team to develop individualized care plans.

Coordinate care with primary care providers, specialists, and other healthcare professionals.

Provide clinical leadership and guidance to nurses, health coaches, social workers, and care coordinators.

Document assessments and care plans in the electronic medical record.

Deliver care in accordance with client policies and applicable state and federal regulations.

Required Qualifications

Master’s degree from an accredited Nurse Practitioner or Physician Assistant program.

Active North Carolina Nurse Practitioner or Physician Assistant license.

Current Nurse Practitioner board certification or Physician Assistant certification.

National Provider Identifier (NPI), active DEA registration, and current Basic Life Support (BLS) certification.

Valid driver’s license and current automobile insurance.

Ability to travel to patients’ homes and work in a clinic as part of the weekly schedule.

Strong communication, problem-solving, organization, and time management skills.

Ability to work independently and collaborate with an interdisciplinary care team.

Prior clinical experience is not required.

Preferred Qualifications

One year of clinical or home care experience.

Experience caring for older adults and patients with chronic or complex conditions.

Experience using electronic medical records.

Physical Requirements

The role involves standing, walking, sitting, reaching, and handling clinical tools and equipment. Candidates must be able to lift or move up to 20 pounds and perform tasks requiring close vision and focus adjustment.
