This is a full-time opportunity offering a personalized care model, strong collaboration, and a high-touch patient experience in a supportive, low-volume environment.
Responsibilities:
Provide comprehensive primary care including diagnosis, treatment, and chronic disease management
Conduct in-depth, personalized visits focused on education and preventive health planning
Order and interpret labs, imaging, and diagnostic tests
Develop individualized care plans aligned with patient goals and lifestyle
Perform in-office procedures such as injections and wound care
Deliver concierge-level service with same-day access, proactive follow-up, and care coordination
Collaborate closely with physicians and care team to ensure seamless patient experience
