Compensation & Benefits
Annual Salary: $120,000 – $140,000, commensurate with experience
Bonus: Performance-based incentive plan

What We’re Looking For
We are seeking a compassionate, detail-oriented Nurse Practitioner (NP) or Physician Assistant (PA) with a strong commitment to patient care. Core Responsibilities
Provide comprehensive outpatient primary care to patients across all age groups
Conduct physical exams, obtain medical histories, and perform diagnostic evaluations
Manage chronic conditions such as diabetes, hypertension, asthma, and more
Design individualized treatment plans aligned with patient goals and clinical guidelines
Educate patients and families on disease prevention, management, and wellness
Collaborate with physicians, care coordinators, and clinical support staff
Document patient encounters accurately and promptly in the electronic health record (EHR)
Maintain current knowledge of clinical guidelines and best practices
Participate in continuing education and meet all licensure and certification requirements
Minimum Qualifications
Active and unrestricted Arizona license as a Nurse Practitioner or Physician Assistant
Valid DEA license without restrictions
Eligible for credentialing with managed care and commercial insurance plans
