Nurse Practitioner for our Skilled Nursing Facilities

Compensation: $105,000 - $130,000 Annual Salary

We are looking for a dedicated Nurse Practitioner to serve as the primary care and rounding provider across our skilled nursing facilities in the Toledo and Paulding, OH area. This is a full-time, in-person position, Monday through Friday, 9 to 5, with no nights, no weekends, and no on-call. You will round between two nearby facilities, managing a panel of long-term care and geriatric patients while working independently from facility staff.

Qualifications:

- Active Ohio NP license (FNP or AGNP) in good standing

- Experience in skilled nursing, long-term care, or geriatric primary care preferred

- Comfortable rounding independently across multiple facilities

- Strong assessment, documentation, and EMR skills

Responsibilities:

- Serve as the primary care and rounding provider for patients across the facilities

- Conduct assessments, manage chronic conditions, and adjust care plans

- Document visits accurately and maintain regulatory compliance

- Participate in patient care meetings and coordinate with the interdisciplinary team
