Demonstrates knowledge of pain management and symptom control, educates patient/family on physical, emotional and psychosocial aspects of care, disease process, symptom and pain management, and signs and symptoms of death. Makes timely home/facility visits to observe and report any change in patient condition, review and contribute to the plan of care, and collaborates with RN or physician to modify plan of care based on effective and timely review of patient and family needs.
