Nurse Practitioner

Central Health Management, Inc. is seeking qualified Nurse Practitioners for a contracted role for 7 days on/off providing primary care in an acute inpatient rehabilitation center near York, PA.

This 1099 role is expected to see between 16 and 20 patients per day and will average around $170,000 annually. The schedule is for 7 days on/7 days off and will take limited call when on.

In this role, you will coordinate care with the physician to ensure quality health outcomes for each patient. Evaluate patient's progress and any changes in condition. Maintain charting in a timely manner. Work closely with the nursing staff in the center to ensure patient safety, comfort and care. Complete discharges and recertifications in coordination with the physician. Educate patients on care transition and follow up with their PCP.

To qualify for this opportunity, you will have:

• An active and clear NP license to practice in Pennsylvania
• An active and clear DEA license to prescribe in Pennsylvania
• MSN or DNP degree from an accredited Nursing school
• Acute care or rehabilitation care experience.
• The ability to work 7 days on/7 days off
• Exceptional patient communication, interaction and care.
• Collaborative skills
