We are seeking a dedicated Licensed Practical Nurse (LPN) to join our home health team. The ideal candidate will provide skilled nursing care, patient education, and support to clients in their homes while collaborating with physicians, RNs, and other healthcare professionals.
Responsibilities
- Deliver skilled nursing care in accordance with physician orders and care plans.
- Monitor patient health, administer medications, and perform treatments.
- Educate patients and families on disease management, medications, and home care techniques.
- Document patient progress and maintain accurate medical records.
- Communicate effectively with the interdisciplinary team to ensure continuity of care.
- Provide compassionate support to patients and families.
Qualifications
- Current LPN license
- Minimum of 1-3 years nursing experience (home health preferred).
- Strong clinical assessment and communication skills.
- Ability to work independently and manage time effectively.
- Reliable transportation and valid driver’s license.
