Position Overview

We are seeking a Nutritionist to provide nutrition education, lifestyle counseling, and patient support across the lifespan. This role helps improve health outcomes by offering culturally responsive guidance, supporting chronic disease management, and collaborating with the care team on preventive health initiatives. The Nutritionist also participates in group education, population health efforts, and quality improvement activities.

Note: This position does not provide medical nutrition therapy (MNT) unless performed under the supervision of a licensed Registered Dietitian, per Oregon regulations.

What You'll Do

  • Conduct basic nutrition assessments, including dietary intake and lifestyle factors
  • Provide individualized nutrition education and counseling for healthy eating, weight management, and chronic disease prevention
  • Support care plans developed by licensed providers and/or Registered Dietitians
  • Provide education for conditions such as diabetes, hypertension, obesity, GI concerns, and maternal/child health within scope of practice
  • Facilitate or co‑facilitate group classes, workshops, and community outreach events
  • Collaborate with primary care, behavioral health, care coordinators, and nursing staff
  • Document all encounters in the EHR according to organizational standards
  • Participate in care team huddles, case reviews, and interdisciplinary planning
  • Maintain current knowledge of nutrition science and evidence‑based guidelines
  • Assist with quality improvement projects, data tracking, and reporting
  • May supervise or mentor interns or support staff depending on qualifications

What We're Looking For

Required:

  • Bachelor's degree in Nutrition, Public Health, Health Education, or related field
  • Knowledge of evidence‑based nutrition principles and health behavior change
  • Strong communication and patient education skills
  • Ability to work effectively in a team‑based, patient‑centered environment
  • Current CPR certification (or ability to obtain within 90 days)

Preferred:

  • Experience in outpatient, community health, or primary care settings
  • Bilingual English/Spanish
  • Experience supporting chronic disease management programs (e.g., diabetes education)
  • Familiarity with Federally Qualified Health Centers (FQHCs)
  • CHES, CDCES, or other relevant certification
