Nexus at Berwyn is seeking a dedicated and compassionate Occupational Therapist (OT) to join our rehabilitation team on a full-time basis. The Occupational Therapist will provide high-quality therapeutic services to residents, helping maximize their safety, independence, functional performance, and quality of life.

Occupational Therapist Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans.
- Develop personalized treatment programs using a variety of therapeutic techniques.
- Provide treatment designed to maximize safety and performance with activities of daily living (ADLs) and instrumental activities of daily living (IADLs).
- Implement treatment in accordance with each resident’s established plan of care.
- Monitor resident responses to interventions and modify treatment approaches as clinically appropriate.
- Collaborate with nursing staff, physicians, therapists, residents, families, and other members of the interdisciplinary team.
- Educate residents, families, and caregivers regarding treatment techniques, functional mobility, safety, and ongoing care needs.
- Complete evaluations, progress notes, treatment documentation, and other required records accurately and on time.
- Ensure all documentation and services comply with applicable company, state, and federal requirements.
- Participate in interdisciplinary care planning and contribute to positive resident outcomes.
- Maintain current knowledge of occupational therapy practices and treatment techniques.
Occupational Therapist Qualifications
- Active and valid Illinois Occupational Therapy license required.
- Bachelor’s or advanced degree in Occupational Therapy from an accredited program.
- SNF/long-term-care experience preferred.
- Current knowledge of occupational therapy treatment practices.
- Strong communication, organization, and interpersonal skills.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Knowledge of Net Health Electronic Documentation System preferred.
- Commitment to providing compassionate, resident-centered care.
